                       Medical Coding is the transformation of healthcare diagnosis, procedures, medical services and equipment into universal medical alphanumeric codes. The diagnoses and procedure codes are taken from medical record documentation such as transcription of physician's notes and lab reports. The title of the ICD is formally the International Statistical Classification of Diseases and Related Health Problems. 

                           The ICD was initially created as a system of diagnostic codes for classifying diseases, offering a wide range of indications, symptoms, abnormal findings, complaints, social contexts, and external causes of harm or sickness that are classified in a variety of subtle ways. This system's goal is to assign a defined code, up to six characters long, for each of these situations, mapping them to the associated generic categories and specific variations.
